What to bring with you for your appointment:

Please have your insurance card with you for your appointment. You may also bring completed patient informational forms and a list of all current medications you are taking, including non-prescription medicines. New patients will need to complete the HIPAA form, Patient Registration and Health history forms.

Medical Records Release

Before requesting your medical records, please speak with your provider to know specifically what records will be needed. An independent company provides the records and a charge for this service is involved. Ordering only the records you need, will be a more economical way to transfer your records. Please call our office for more information.
